Mosaic Health

Mosaic Health is a not-for-profit network of Community Health Centers with eight service locations that provide primary medical, dental, and behavioral health care across several counties in Upstate New York, including Herkimer, Livingston, Monroe, Oneida, Ontario, Wayne, and Yates Counties. We also provide school-based portable dental services at almost 200 schools throughout the Finger Lakes region and in Oneida County. Mosaic Health is dedicated to providing access to quality health care for all, regardless of financial, cultural, or social barriers.

Mosaic Health is federally funded to treat the underserved and underinsured. We served approximately 20,000 patients at locations in Ilion, Lyons, Mount Morris, Rushville, and Utica New York in 2018.

Advancement opportunities exist throughout our growing network. Our recent name change reflects our strong patient commitment.
