Our Vision:
Nestled in the peaceful neighborhood of 54th and LaSalle Streets, Homestead Rehabilitation Center offers a thriving environment for short-term rehabilitation, skilled nursing extended care, memory support, respite and end of life care. Homestead accepts admissions seven days a week, including holidays. Your rehabilitation begins right away with a Therapy Assessment within 24 hours of admission with physical, occupational and speech therapy offered every day of the week. With Homestead’s onsite nurse practitioner and 24-hour RN coverage, your needs are always our priority. Visit our neighborhood today!
Our Mission:
Homestead Rehabilitation Center is an integral part in the continuum of care. Our delivery of services is adaptable to the provision of present and future care needs of our customers.
Our goal is to provide a highly skilled and supportive environment through which we can foster dignity, independence and self-determination to the fullest extent of our customers.
To meet the needs of our customer, our employees representing many disciplines and working together as an integrated clinical team to exceed your expectations.
We are firmly committed to providing our services within available resources consistent with achievable goals.
Our History:
When The Homestead opened in 1963, it sat alone on the grounds of a dairy farm, with only the house that was the “homestead,” and a few newer homes to the north. Fifty-Sixth Street was a gravel road and kids were dancing to the Twist, the average house cost $3100 and troll dolls were the rage. At the southern and eastern most part of town, Homestead was surrounded by cornfields. The home that was located where Homestead is now, was relocated to Prescott Street and is still there today. The Homestead served 100 people at the time and staff used to purchase fresh farm eggs from the house on the corner at 54 and LaSalle Streets.
1992: Memory Care Community opened – secure, specialized care for people with Alzheimer’s and Dementia
2013: Celebrated 50 Years with Expansion to add new private rehabilitation community with 12 private rooms. Renovations were also completed in 2013 to update common areas.
2014: Received the National Quality Bronze Award from the American Health Care Association. Facilities must achieve the Bronze level, before they can reach the Silver or Gold levels.
Today: We employ over 200 people to provide comprehensive rehabilitation, skilled nursing care, memory support, respite and end of life care.
