About Primary PartnerCare®
Primary PartnerCare® is a physician-driven organization with doctors in Nassau, Suffolk, Queens, and New York Counties. We were founded on the belief that private practice primary care doctors in partnership with their patients produce optimal health. Established in 2012 to help independent physicians meet the demands of healthcare reform, we provide tools and resources for doctors to remain independent and thrive.

Who We ArePrimary PartnerCare® is a physician-driven organization founded on the belief that private practice primary care doctors in partnership with their patients produce optimal health. Our physicians achieved the highest Medicare quality score in New York for 2015 and 2016 at 97%, and achieved Medicare Shared Savings in 2017. We offer physicians turnkey accountable care solutions to actively participate in the Medicare Shared Savings Program and similar value-based health plan contracts. These solutions allow physicians to maintain autonomy and restore the joy of medicine to their practice. At Primary PartnerCare®, efforts and resources are totally focused on a healthier and happier future for both our doctors and their patients.
