J&M Family Management, headquartered in Grapevine, Texas, is a family-owned business with a passion for senior care. This focus led to the creation of Autumn Leaves® nearly 20 years ago, a superior standalone memory care community designed specifically to meet the needs of people living with Alzheimer’s and other forms of dementia.
The portfolio now consists of nine Autumn Leaves memory care communities in Illinois, Oklahoma, Wisconsin, and Texas as well as one dual-focused independent and assisted living community in Corinth, Texas.
Today the company continues as a family-owned and operated business focused on the same mission and values that have created an excellent track record of care for our residents and peace of mind for investors.
