Ontrak, Inc. helps people with behavioral health conditions that result in serious medical issues. We use advanced analytics to find people in need, and connect with them on their terms. We treat them in a holistic, evidence-based program that includes nurse-led care coaching, face-to-face community-based resources, and behavioral health care. Through the OnTrak program, these people become healthier, resilient and strong. Leading health plans contract with us to help their members regain control of their health and their lives.

Member Engagement

Our impact starts here. Without our Member Engagement Specialists we never enroll a new member. The MES changes the trajectory of our business. This is more than a sales role – this is a role that takes a unique approach in helping care avoidant health plan members see that we can help them if they enroll in our programs. Our ability to change lives starts with the Member Engagement Specialist. This is an empathetic sales role serving people with a product you can stand behind.

Care Coaches

On a daily basis our Care Coaches are helping our care avoidant members see that they can do it. They encourage and empower members by coming alongside them setting up small milestones that make lifechanging impact.

Digital Technology

Our Digital Technology team is breaking new ground in Artificial Intelligence and distributed infrastructure for working professionals. Digital Technology colleagues work collaboratively with our front-line teams to discover new ways to work and to learn how to leverage the best technology to bring best in class resources to our staff, members and clients. Our team comes together to use their skills to do more meaningful work.
