About Modoc Medical Center
Our mission at Modoc Medical Center (MMC) is to elevate the provision of health care by creating a partnership with our patients, employees, businesses, and the community. Embedded in this mission statement is the fundamental belief that our success will depend on our ability to provide quality healthcare services to our patients and establish processes and a culture that will put our patients at the center of what we do.
Our ability to work together as a team in delivering quality health care services will largely impact our ability to accomplish our mission by placing the patient first and providing each other with the support we need as a team to deliver the best health care possible.
In addition to keeping the focus on our patients and the health care we are delivering, we act according to the responsibility we have to members of this community to be good stewards of the public funds that support this organization and to make sure we are operating as efficiently and effectively as possible so that the necessary financial resources are available to support what we need to do to continually improve the services we provide to the community.

MMC has received approval from the USDA on a loan that will be used to construct our new facility. The new facility project includes the construction of a new hospital, clinic, ambulance shed, physical therapy department, and other clinical and non-clinical departments on property owned by MMC south of Highway 299.
The loan with the USDA was approved for $35,130,000 and will have a 40-year term. We will contribute the remaining $5,483,900 of our own funds to reach the total anticipated project budget of $40,613,900. Though this is subject to a number of conditions outlined by the USDA, Administration anticipates being able to meet them all.
We are currently seeking a construction management firm. A recommendation for this is expected to be made to the Board by the end of July. After a firm is engaged, we will begin work on selecting a design-build firm.
MMC is hoping that a decision on a design-build firm will occur by the end of the year. After that, work will begin on a complete set of drawings to be submitted to the Office of Statewide Health Planning and Development (OSHPD) for
approval. Approval from OSHPD must be received before construction begins. Though there is still a lot of work to be done for this project, we are grateful that we were able to take such a big step in the right direction.

In order to provide the best possible patient care and in an effort to better our facility, MMC conducts several patient surveys. The goal of our surveys is to provide patients with the best possible experience while they are in our care. Your feedback is vital for our improvement.
One step that our Family Practice Clinic has taken to improve our processes is to verify patient contact information. We need to have updated information so that we can contact patients for upcoming appointments, to set referral appointments and in order to provide patients with test results. The Clinic is double checking all patient information to better our services.
Another step MMC is taking in improving processes is by following up with ER patients after their ER visit. The nursing staff is contacting patients to make sure that they are doing well and to answer any questions the patient may have after being discharged. This is one more step we are taking to ensure that our patients' opinions are not only being heard but are also being acted upon.
Another way in which we look to hear from our patients is through Patient Satisfaction Surveys on all patients admitted to the hospital. Though this is a requirement from the Centers for Medicaid and Medicare, our most recent Survey results placed our hospital rating at 100.0% overall! MMC also rated 100% for cleanliness/quietness, 93.3% for communication with Doctors and 100% of patients would recommend MMC. These scores are all above the National Average. That being said, we will always have things to improve upon. We are currently working on improving our care transitions at the hospital, as well as modifying our pain management techniques. Patient feedback, YOUR feedback, is valuable to us and helps us to improve care for our patients.
We have also received helpful information from our Clinic Surveys. These indicate that 88% of our patients felt the Clinic's front office staff attitudes were "Beyond Expectations" and 92.8% of the Clinic patients rated their overall experience at the Clinic as also being "Beyond Expectations." MMC is moving forward in the quality arena.
Positive Community Feedback:
I know where to go if I need anything at all.
Dr. Lai is kind and very intelligent. He found the problem and fixed it.
A patient came in with a dislocated hip, which Dr. Lai put back into place. The patient then went to an orthopedic surgeon and the surgeon said that Dr. Lai did an amazing job putting the hip back.
Our focus is on continuous improvement of quality care, but we need your input. If you have any questions or comments regarding quality improvement, contact our Quality Improvement Coordinator:
